Recommendations for the post-operative diet

A patient who has had a bariatric operation needs several months for his/her digestive canal to adapt to the new anatomy that was created through the operation.

Therefore, strict dietary rules must be followed, especially at the beginning of the post-operative period. These rules also depend on the type of surgery that was received.
In general, the precautions that initially need to be taken relate to the reduction of the stomach's capacity. To cope with this, the diet will be liquid to half liquid in the first month.
It is imperative that it is poor in sugar, because food arrives prematurely into the small intestine (especially with a bypass and DS), which results in "the dumping syndrome effect" as described earlier.

Finally, vitamin supplements and oligo-elements will be compulsory for life for patients who have had a duodenal switch operation, and to a lesser extent for those who have had a bypass.



Gastric bypass diet :


LIQUID FOOD AFTER GASTRIC BYPASS
 
1.  During the first month following the operation, food will be solely based on liquids.
 
Tips :
 
v    Drink with small sips, the quantities of liquid should not exceed 100ml ( approx. ½ glass).
 
v    Vary the liquids: non-sugared skimmed milk products, diluted fruit juices, vegetable juices, soups, coffee, tea. Avoid products with a high fat and sugar content (whipped cream, milkshakes, non-diluted juices).
 
v    Limit milk to 2 glasses a day and give priority to soya milk and milk poor in lactose (sold in supermarkets).
 
v    Foresee, daily dietary supplements rich in proteins sold either at the pharmacy: Protical, Phytodiet, Dynaline; or in supermarkets and chemist shops: Hyperprotéinés of Gaylord Hauser, Easy Body, Proslim
  
NB : you can also enrich your liquid preparations with proteins by adding an egg or skimmed milk powder, 1 tbs. of liquidised ham or meat or hyperproteined powder (Protifar) .
 
v    Avoid sugary and carbonated drinks.
v    Regularly take your multi-vitamin supplements.
 
 
2)   After the first month,  the second food re-introductory phase consists of a diet based on 
-        potatoes and vegetables mashed together: portion of max. 100-150g (2 to 3tbsp. per meal)
-        dairy-based desserts, compotes without sugar
-        soft foods: soft white bread, raw minced meat such filet américain, fish, etc
before moving onto solid food, vary with very small quantities.
Continue to avoid heavily sugared products than can make you feel sick!
(Dumping type): chocolate, ice creams, syrups, pastries, etc.
 
FOOD PLAN : LIQUID DIET 1st month
 
Before breakfast
 
Coffee or tea without sugar in little sips             200ml
With milk or lemon
An artificial sweetener is allowed
 
Breakfast
1 liquid dairy product, for example:
skimmed quark (possibly diluted with milk and sweetened with a sugar substitute) 
or 2 petits suisses (fromage frais)

Lunch : divide in 2 parts or take small mouthfuls
 
1 bowl of vegetable soup enriched with:            300ml

Afternoon snack
 
1 dairy-based dessert or protein drink

Supper
 
½ bowl enriched soup (idem noon)                    150ml
½ hour later: rest of enriched soup                   150ml
 
 
Snack 8 p.m.
1 liquid dairy product, for example :
« Light » dessert (Vitalinea, Sveltesse)              150ml
 
 
PHASE 2 : SOFT DIET
                                                                                            
The first days, your food reintroduction plan will consist of a diet based on purées (portion: max. 100-150g per meal, and compotes. Then soft foods: soft white bread, raw minced meat such as filet américain will be re-introduced, before progressing onto solid food, varied in very small quantities.
 
Tips:
 
1)       Break up your meals: have 4 to 5 meals at an average temperature (neither ice cream nor hot food or drinks) observing intervals of 2 hours between meals.
 
2)       Eating soft food is easier at the beginning. But then get in the habit of chewing well, when you eat harder foods (such as raw vegetables), being careful not to swallow unless every mouthful is chewed well.
 
3)       Avoid drinking during meals: take your drinks in between meals. Initially avoid carbonated drinks that make you feel heavy and bloated. Ideally, 6 to 8 glasses of water per day are advised.
 
4)       Manage stopping in time: especially do not force yourself; as from the first signs of heaviness, bloating or nausea, stop eating.
 
5)       Avoid heavily sugared foods: desserts, jam, honey, syrup or non-diluted fruit juice. Eat low-fat foods.
 
6)        Be moderate in the consumption of milk and products rich in lactose: yoghurts, dairy desserts, quark, excess consumption of which might cause sickness. Limit to 2 products per day.
 
7)       Take your vitamin supplements regularly and replenish your protein intake by taking daily a protein enriched product.

 
 

 

CHOOSING YOUR FOOD WELL
 
Choose your food well so that you have a balanced diet and avoid deficiencies. 
For proteins : Eat daily two products as desired :
 
              either meat: 50g/j (raw minced steak, minced steak, veal meatloaf, or chicken, or turkey) but avoid hard and fibrous meat: casseroles, stews, pot au feu (French beef stew)
               or fish, shrimps, scampis: 50g/j
               or lean deli meats: 50g/j (lean ham, chicken or turkey filet)
              or an egg 
 
ADD 1 hyperproteined product, the type that comes in a sachet and is used for a protein diet, or Profile, Fortimel, Clinutren that you break up in small parts over the day. (Important! do not drink it in 1 go.)
 
As a snack, a minimum of 3 non-sugared light dairy products, to be chosen according to your personal tolerance
 
               either milk (150ml)
               or yoghurt (125g)
               or pudding (150ml)    
               or quark (50g)
               or soft or hard cheese (20 to 30g)
 

Do not forget to consume every day fruit and vegetables (possibly liquidised at the beginning):
Start by choosing soft vegetables with few fibres (such as cooked endives, cooked cauliflower, cooked lettuce, carrots, courgettes, fine green beans, artichoke bottoms, asparagus tips, the white of leaks, chicory, spinach, peeled tomatoes, that at first you eat cooked).
  
Raw vegetables will be tried out very carefully after the cooked vegetables (remember to chew well).
Fibrous or less digestible vegetables that should be avoided during the first month are:
sauerkraut, cabbages (except for cooked cauliflower), black salsify, onions, celeries, mushrooms, bell peppers, turnips, broad beans, lentils, and dried vegetables.
 
Regarding fruit: eat them cooked at the beginning (non-sugared compote). When you eat them raw, start with soft fruit: well ripened pears, bananas, peaches, and kiwis.
Avoid fruit with pips (such as grapes) and make sure you remove the skin (pears, peaches, apples).
 
Food you should not eat (that risks preventing weight loss or that will not be tolerated):
 
-         sugared drinks (lemonades, non-diluted fruit juice, carbonated drinks)
-         alcoholic beverages
-         chocolate, sweets
-         pastries, whipped cream, ice creams
-         non-light mayonnaises and sauces
-         crisps
Namely all foods with a high sugar and/or fat content.
 
EXAMPLE OF AN EATING DAY (for the first month)
 
 
BREAKFAST
 
-         Coffee or tea (non-sugared) to drink before the meal
-         1 little sandwich (possibly toasted) or 1 cracker or 1 biscuit
-         low-fat spread
-         low-fat cheese or low-calorie jam
 
OR
-         Cereals: Corn Flakes or Special K, Fitness with a little semi-skimmed milk
 
 
10 A.M. SNACK
 
-         A third to half of a HYPERPROTEIN product drink, such as Fortimel (pharmacy)
          or 1 protein sachet
-         Artificially sweetened dairy product (yoghurt or quark or petit suisse)        
                                              
OR
-         Half a piece of fruit or compote without sugar
 
 
NOON
 
-         Fish: 50 to 70g or 50g of minced meat or chicken or an egg
-         Cooked or raw vegetables (1 tbsp.)
-         Potatoes (1 small one) or mash (1tbsp.) or rice (1tbsp.) or pasta (1tbsp.)
 
 
4 P.M. SNACK
 
-         1 Artificially sweetened dairy product or half a piece of fruit
-         and the rest of the protein product drink
 
EVENING
 
-         1 small sandwich or 1 cracker or 1 biscuit
-         Low-fat spread
-         Low-fat cheese, or ham or tuna or raw minced steak: 50g
 
 
9 P.M. SNACK
           
-         1 Dairy product if tolerated (yoghurt, quark, gervais or petit suisse)
 
DRINKS
 
-         Coffee or tea, still water (carbonated water or low-calorie lemonade after 2 to 3 weeks after the operation).
